Access personalized adult primary care through a Direct Primary Care membership without insurance billing or long-term contracts.

Seniors (65+)$175 / month

Receive comprehensive adult primary care through a monthly membership designed to support ongoing health needs and continuity of care.

Enrollment Fee$100 one-time

A one-time enrollment fee is required at signup to establish membership and complete onboarding.

Membership is billed monthly in advance, offered month-to-month, and requires a 30-day written notice for cancellation. Membership fees are non-refundable.

What Your Membership Includes

Membership includes access to adult primary care services when clinically appropriate, including:

  • In-person primary care visits
  • Telehealth visits by video or telephone
  • Preventive and wellness evaluations
  • Acute illness assessment and treatment
  • Chronic disease management
  • Medication management within Nurse Practitioner scope of practice
  • Prescription refills for non-controlled substances
  • Secure electronic messaging related to active care
  • Care coordination and referral support

Services are provided on a reasonable and medically appropriate basis.

What Membership Does Not Include

The following services are not included in the membership fee:

  • Laboratory and pathology services
  • Diagnostic imaging
  • Specialist consultations
  • Emergency department or hospital services
  • Medications dispensed by pharmacies
  • Durable medical equipment
  • Procedures beyond routine primary care
  • Disability, legal, or occupational forms
  • Services outside Nurse Practitioner scope of practice
  • Any service not expressly listed as included
Certain services are specifically excluded from membership, including:
  • Chronic opioid therapy
  • Long-term benzodiazepine management
  • Routine stimulant management
  • Pain management requiring specialty oversight

These services fall outside the scope of membership and require alternative or specialty care.

Patients are informed in advance when a service is not included in membership.
