Is membership required to receive care?
Yes. Lynch Healthcare provides services through a Direct Primary Care membership. Membership is required before scheduling routine visits.
What should I bring to my first visit?
Please bring a government-issued ID, a list of current medications, and any relevant medical records. Insurance cards are not required.
Can you help manage chronic conditions?
Yes. Lynch Healthcare provides ongoing management of chronic conditions through individualized care plans and regular follow-up when clinically appropriate.
How often will I need to be seen?
Visit frequency is determined based on your individual health needs and clinical appropriateness, rather than a fixed schedule.
Do you offer telehealth visits?
Yes. Telehealth visits are available for certain concerns when clinically appropriate and do not replace all in-person care.
Do you provide vaccinations?
Lynch Healthcare provides preventive care guidance, including education and counseling on recommended vaccinations. Vaccine administration and supply may occur through external providers or pharmacies and are not included in membership.
How can I transfer my medical records?
You may request records from your previous provider and have them sent directly to Lynch Healthcare. Assistance is available if needed.
