At Lynch Healthcare, chronic disease management is part of our Direct Primary Care (DPC) membership for adults age 18 and older.

Living with a chronic condition such as diabetes, hypertension, asthma, thyroid disorders, or heart disease requires consistent monitoring and thoughtful care. Our goal is to help you maintain stability, prevent complications, and improve your quality of life through a structured, personalized approach.

As a Nurse Practitioner-led adult primary care practice in Wall Township, New Jersey, we provide ongoing management within scope of practice and coordinate specialty referrals when necessary.

What Chronic Disease Management Includes

Your membership provides access to medically appropriate chronic care services such as:

  • Regular follow-up visits (in-person or telehealth when appropriate)
  • Blood pressure and vital sign monitoring
  • Medication management within NP scope of practice
  • Prescription refills for non-controlled medications
  • Review of laboratory results (performed by outside facilities)
  • Lifestyle counseling (nutrition, physical activity, sleep, stress management)
  • Risk factor modification planning
  • Care coordination and referral support when specialty care is needed
  • Secure messaging related to active care plans

Services are provided based on clinical judgment and medical necessity.

Conditions Commonly Managed

Diabetes Management

We monitor blood sugar control, review medications, and provide education to support healthy lifestyle adjustments. Laboratory testing is coordinated through outside facilities when indicated.

Hypertension (High Blood Pressure)

Routine monitoring and medication adjustments help reduce long-term cardiovascular risk. We also discuss heart-healthy lifestyle strategies to support blood pressure control.

Asthma and Mild-to-Moderate Respiratory Conditions

We monitor symptoms, review inhaler use, and adjust treatment plans when appropriate. Patients requiring advanced pulmonary care are referred to specialists.

Cardiovascular Risk Management

We assess risk factors such as cholesterol levels, blood pressure, and lifestyle contributors. Diagnostic imaging or advanced cardiac testing is referred externally when indicated.

What Is Not Included

The membership does not include:

  • Laboratory testing or imaging
  • Specialist consultations
  • Hospital or emergency services
  • Advanced pain management
  • Chronic opioid therapy
  • Long-term benzodiazepine management
  • Routine stimulant management
  • Specialty-level cardiac or pulmonary procedures

Patients are informed in advance when outside services are recommended.

  • We do not bill third-party insurance
  • Controlled substances are not guaranteed and are prescribed only when clinically appropriate and legally permitted
  • Telehealth is used when appropriate but does not replace necessary in-person evaluation
  • Care is provided within the Nurse Practitioner’s scope of practice under a collaborative physician agreement as required by New Jersey law
